Weekly COVID Update

Dear Phoenix Center’s Residents, Families, Staff, and Visitors:

As of today, we have 2 active (+) employee cases, quarantined at home and 0 active (+) resident cases. Facility will continue to monitor all staff and residents for Covid-19 symptoms and will take any appropriate steps necessary if deemed to be symptomatic. The facility continues to follow all CDC and DOH recommendations and continues to report specific numbers to the DOH on a daily basis. Facility will continue to test staff and resident’s weekly.

We recommend having LIMITED visitation while we are in an outbreak to reduce the transmission of COVID-19.

SUGGESTED HOURS FOR VISITATION ESPECIALLY FOR THE WEEKENDS ARE 10 AM TO 8 PM.

If you plan to visit after suggested hours, please notify your social worker or front desk prior to visitation during the day.

**Rapid test must be completed upon arrival and full PPE must be worn at all times during visitation with loved one**

PLEASE DISCARD DISPOSABLE GOWNS INSIDE YOUR LOVED ONE’S GARBAGE PRIOR TO LEAVING UNIT WHEN VISITATION IS OVER

Failure to follow the policy may result in NO future visitation.

VACCINE UPDATE: Our Vaccine Clinic is held monthly. We are now able to vaccinate children from age 5 to 11 with Pfizer Vaccine. We strongly encourage all family members to have all eligible residents vaccinated! If you have any questions on the vaccine, you can speak to our ID Doctor or IP Nurse. Unit managers or your social worker will be reaching out to you to receive consent.

OTHER UPDATE: Dayroom’s activities are open but also subject to change if a new confirmed case if found on the unit. However, we continue to provide 1:1 activities if that occurs!

RENOVATION: We have completed the second-floor pediatric unit! We have started our first-floor pediatric renovation! Our goal is to ensure we are keeping our residents safe during construction. We have a safety plan that is available if anyone wishes to review it or if anyone has any questions, please be free to contact Administration.
