Weekly COVID Update

May 5th, 2023


Dear Phoenix Center’s Residents, Families, Staff, and Visitors:


As of today, we have 0 active (+) employee cases and 0 active (+) resident cases. Facility will continue to monitor all staff and residents for Covid-19 symptoms and will take any appropriate steps necessary if deemed to be symptomatic. The facility continues to follow all CDC and DOH recommendations and continues to report specific numbers to the DOH on a daily basis. Facility will continue to test staff and resident’s weekly. 


SUGGESTED HOURS FOR VISITATION ESPECIALLY FOR THE WEEKENDS ARE 10 AM TO 8 PM.


UPDATED INFORMATION:
FAMILY/VISITOR MUST PEFORM RAPID TEST ON-SITE PRIOR TO VISITING BUT AS WE ARE ENTERING INTO FLU SEASON AND THE RISE OF COVID-19 THIS SEASON:
1.    1st FLOOR PEDIATRIC VISITORS MUST WEAR FULL PPE PRIOR TO ENTERING LOVED ONE’S ROOM
2.    GERIATRIC VISITORS ARE REQUIRED TO WEAR MASK AT ALL TIMES
MASKS MUST BE WORN DURING VISITATION AT ALL TIMES.
IF YOUR LOVED ONE IS ON PRECAUTION, THERE WILL BE A SIGN ON THEIR DOOR AND WILL HAVE INSTRUCTIONS TO WEAR DISPOSABLE GOWN AND SHIELD THAT WILL BE READILY AVAILABLE ON DOOR CADDY TO USE. PLEASE DISCARD DISPOSABLE PPE INSIDE THE ROOM PRIOR TO LEAVING ROOM.


Failure to follow the policy may result in NO future visitation.


VACCINE UPDATE: Our Vaccine Clinic is held monthly. We are now able to vaccinate children from 6 months and above! We strongly encourage all family members to have all eligible residents vaccinated! If you have any questions on the vaccine, you can speak to our ID Doctor or IP Nurse. Unit managers or your social worker will be reaching out to you to receive consent.


RENOVATION: We have completed the second-floor pediatric unit! We have started our first-floor pediatric renovation! Our goal is to ensure we are keeping our residents safe during construction. We have a safety plan that is available if anyone wishes to review it or if anyone has any questions, please be free to contact Administration.

Our staff will continue to work hard to ensure we are keeping your loved one safe and themselves from COVID-19. Hope everyone has a great weekend!
